  • Loss of life because of AIDS (as defined by the WHO)
  • Loss of life due to undisclosed critical/terminal illness
  • Death due to a terminal illness diagnosed within the last 18 months of the policy tenure
  • Individuals working as pilots, professional sport activists, police or defence personnel, or cabin crew

For Terminal Illness

  • Terminal illness claim form
  • Document from a physician stating the detailed diagnosis and the cause of terminal illness, and the confirmation that the life assured has a life expectancy of 12 months or less from the date of claiming Terminal Illness benefit
  • If the terminal illness was confirmed overseas, original certified copy of the medical report attested by the relevant embassy present in the resident country or another medical report from the residence country
  • Copy of Emirates ID or passport with residence visa page

For Permanent Total Disability 

  • Disability claim form
  • Disability claim from physician (mention the degree of disability)
  • For proof of ‘Permanent Disability’, present a medical report or disability certificate from an authorised physician or a medical report authorised by the medical board to assess disability after 12 months from the date of disability 
  • If the disability is confirmed overseas, the relevant embassy in UAE needs to provide an attested disability certificate
  • Emirates ID or passport with residence visa page

For Critical Illness

  • Critical illness form
  • Medical/hospitalisation report from a licenced medical practitioner detailing the diagnosis of the critical illness (date when the ailment occurred)
  • Confidential medical questionnaire 
  • If the critical illness is confirmed overseas, the relevant embassy in UAE needs to provide an attested medical report
  • Emirates ID or passport with the residence visa page of the claimant
